At Elder Folks, we’re here to guide you through the next steps clearly, calmly, and with care. Support at Home is designed to help you stay in the place that feels most like you, your home. Because ageing well starts with feeling safe, supported, and in control of what happens next.

What is Support at Home?

From 1 November 2025, the Australian Government is introducing a new program called Support at Home, replacing the current Home Care Packages and Short-Term Restorative Care programs.

Who it’s for

You may be able to access Support at Home if you are:

  • Aged 65 years or older
  • Aged 50 years or older (for Aboriginal and Torres Strait Islander people)
  • Aged 50+ and at risk of homelessness

If you’re not sure where you fit, we can help you work that out.

What support can look like

Support at Home includes 8 levels of funding, so your care can match your needs as they change over time. Funding currently ranges from around $11,000 to $78,000 per year, depending on your situation. We’ll help you understand what this means for you, clearly and simply and work with you to build support around what matters most in your daily life.

Health and wellbeing support

  • Nursing care
  • Allied health (like physiotherapy or occupational therapy)
  • Nutrition support
  • Care coordination

Help with daily living

  • Personal care (like showering or getting dressed)
  • Social support and connection
  • Transport to appointments or activities
  • Respite for carers

Support around your home

  • Cleaning and household help
  • Home maintenance and small repairs
  • Meals and meal preparation

Every plan is different, because every person is.

Extra support when you need it

There may be times when you need a little more help. Support at Home includes short-term options to support you through these moments.

Making your home safer

You may be able to access funding for equipment or home changes—like handrails or mobility aids—so you can move around safely and confidently.

Regaining strength and independence

With Restorative Care, you can access up to 12 weeks of support to help you get back on your feet and stay independent for longer.

Support at the end of life

If needed, additional support is available to help you remain at home with comfort, dignity, and the people who matter most around you.

Understanding Support at Home Funding and your contribution

Support at Home offers 8 levels of funding, allowing your care to be tailored as your needs change over time. Funding currently ranges from approximately $11,000 to $78,000 per year, depending on your individual circumstances.

We’ll help you understand what this means for you in a clear and simple way, and work alongside you to design support that reflects what matters most in your daily life. The amount you contribute towards your care will depend on your financial situation.

If you are a full pensioner, you will not pay for clinical care. However, you may be asked to contribute a small amount towards everyday support and independence services, including:

  • Personal care
  • Social support and community engagement
  • Therapeutic services for independent living
  • Respite care
  • Transport
  • Assistive technology and home modifications
  • Domestic Assistance
  • Home maintenance & repairs
  • Meals

If you are a part pensioner or a self-funded retiree, your contribution will be assessed based on your income and assets. We will guide you through this process step by step, ensuring everything is clear and there are no unexpected costs, just a straightforward path forward.
