Craig is a kind and softly spoken gentleman living alone in a Homes NSW property.

With no known family support and increasing difficulty managing daily tasks, Craig had been quietly struggling. When Elderfolks became involved, it became clear that his situation was more complex than it first appeared, and that he was at significant risk without additional support.

 

The Challenge

Craig was showing signs of cognitive decline, and his overall health was deteriorating.

He had experienced noticeable weight loss, was unable to safely access his bath to shower, and his home environment had declined, with signs of squalor. These factors indicated that Craig was no longer able to live safely without assistance.

Urgent intervention was needed to stabilise his health, improve his living conditions, and ensure his safety and wellbeing.

 

Our Approach

Craig was open to support and engaged positively with the Elderfolks team.

We assisted him to register with My Aged Care and coordinated an urgent ACAT assessment, advocating for a range of care options, including both in-home and residential support.

While Craig initially agreed to explore residential aged care, he later expressed a clear preference to remain at home. Elderfolks respected this decision and focused on securing the supports needed to make this a safe and sustainable option.

Given Craig’s vulnerability and lack of informal supports, we successfully advocated for fast-tracked approval of in-home care services.

 

The Outcome

Elderfolks supported Craig to engage a home care provider suited to his needs, ensuring he received consistent, compassionate support.

Services were introduced to address both his immediate and ongoing needs, including a comprehensive home clean, transport to regular medical appointments, access to nutritious meals, domestic assistance, and social support such as shopping and community outings.

An Occupational Therapy assessment was also arranged to identify home modifications and equipment to improve safety and accessibility, with these supports implemented through his care provider.

As a result, Craig’s health, confidence, and overall wellbeing have improved significantly. He is now more connected, better supported, and able to live safely in the comfort of his own home.

Craig’s story highlights how isolation can gradually impact both physical and mental wellbeing.

Without visible support networks, people can quickly become vulnerable—but with the right intervention, this can be turned around.

By taking the time to listen, advocate, and coordinate meaningful support, Elderfolks helped Craig regain stability, dignity, and a renewed sense of independence in his daily life.
